Offer Eligibility:

This rebate offer (“Offer”) is valid only for Canadian residents who:

• Are the age of majority under applicable law.
• Complete a qualifying eye fitting at a participating optometrist location in Canada during promotional period.
• Purchase two (2) boxes of eligible Bausch + Lomb ULTRA® ONE DAY Contact Lenses, Bausch + Lomb Biotrue® ONEday lenses, or (4) boxes of ULTRA® Monthly Lenses as an Essentials Kit (“Qualifying Purchase”) in a single transaction; and
• Submit a valid fitting fee or full eye exam invoice.

Offer Period:

Valid for Qualifying Purchases made between June 1 and August 31, 2026 or while supplies last (“Offer Period”).

How to Claim Your $50 Rebate:

To receive a $50 prepaid Visa® card (issued digitally), you must:

1. Book and complete either a comprehensive eye exam or a contact lens eye fitting at a participating Every Contact Counts optometrist during the Offer Period.
2. Purchase two (2) boxes of eligible ULTRA® ONE DAY Contact Lenses, Biotrue® ONEday lenses, or or (4) boxes of ULTRA Monthly Lenses as an Essentials Kit (“Qualifying Purchase”) in a single transaction; and
3. Upload the following items no later than August 31, 2026.
4. A legible copy of your receipt showing payment for the eye fitting and eligible product purchase. Must be clear and legible to show product purchased and purchase date.
5. A completed rebate form, available from your eye care provider.

Limit:

Limit one (1) rebate per person for any Qualifying Purchase except where prohibited. Offer is not valid in combination with any other offer, rebate, or promotion on the same product.

Rebate Delivery:

Eligible participants will receive a $50 prepaid Visa® card delivered digitally via email to the address provided on the rebate form, within 4-6 weeks of receiving a valid submission. Card is subject to terms and conditions of the card issuer.

Additional Terms:

• Incomplete, illegible, late, or duplicate submissions will not be accepted.
• Offer is void where prohibited or restricted by law.
• Sponsor reserves the right to cancel, modify, or withdraw this Offer at any time without notice.
• Proof of purchase must be authentic and verifiable.
• Digital card must be activated and used in accordance with issuer’s terms.
• Participants are responsible for all applicable taxes, if any, as well as all fees and expenses associated with acceptance and use of the Offer not specified herein as being provided. No returns are permitted. Offer is not transferable, assignable, or substitutable, except in the sole and absolute discretion of Sponsor. No group or organization requests will be honored. Bausch + Lomb reserves the right to modify or replace the Offer with an item of comparable or greater value in its sole and absolute discretion.
